HUBUNGAN PARITAS DENGAN KEJADIAN DROP OUT KONTRASEPSI SUNTIK 3 BULAN (DMPA)

ABSTRACT
Dropout from the 3-month injectable contraceptive (Depot Medroxyprogesterone Acetate [DMPA]) remains a challenge to the Family Planning (FP) program because it increases the risk of unintended pregnancy and reduces contraceptive continuation rates. Ideally, contraceptive users should maintain timely and consistent use of contraception; however, discontinuation is influenced by several factors, including parity, which is closely related to women's reproductive experience, family planning needs, and reproductive goals. Although the association between parity and contraceptive continuation has been reported in previous studies, evidence among DMPA users in the study setting remains limited. This study aimed to analyze the relationship between parity and dropout from the 3-month injectable contraceptive (DMPA) using a correlational analytic design with a retrospective approach based on secondary medical record data. The study population consisted of 241 DMPA users, and 71 respondents were selected using proportionate stratified random sampling. Data were analyzed using the Chi-square test. The results showed that most respondents were multiparous (77.0%) and had not discontinued DMPA use (53.5%). Statistical analysis demonstrated a significant association between parity and dropout from the 3-month injectable contraceptive (p = 0.002; OR = 0.132), indicating that multiparous women were less likely to discontinue DMPA use than primiparous women. These findings suggest that parity is an important factor influencing the continuation of DMPA use and may serve as a basis for strengthening family planning screening and counseling tailored to clients' parity characteristics.
ABSTRAK
Drop Out kontrasepsi suntik 3 bulan (Depot Medroxyprogesterone Acetate/DMPA) masih menjadi tantangan dalam program Keluarga Berencana (KB) karena meningkatkan risiko kehamilan yang tidak direncanakan dan menurunkan keberlangsungan penggunaan kontrasepsi. Secara ideal, akseptor diharapkan mempertahankan penggunaan kontrasepsi sesuai jadwal, namun kejadian Drop Out masih dipengaruhi oleh berbagai faktor, salah satunya paritas yang berkaitan dengan pengalaman reproduksi, kebutuhan, dan tujuan ber-KB. Meskipun hubungan paritas dengan keberlangsungan penggunaan kontrasepsi telah dilaporkan dalam berbagai penelitian, bukti pada akseptor DMPA di lokasi penelitian masih terbatas. Penelitian ini bertujuan menganalisis hubungan paritas dengan kejadian Drop Out kontrasepsi suntik 3 bulan (DMPA) menggunakan desain analitik korelasional dengan pendekatan retrospektif berdasarkan data sekunder rekam medis. Populasi penelitian berjumlah 241 akseptor DMPA, dengan sampel sebanyak 71 responden yang dipilih menggunakan teknik Proportionate Stratified Random Sampling. Data dianalisis menggunakan uji Chi-Square. Hasil penelitian menunjukkan bahwa sebagian besar responden merupakan multipara (77,0%) dan tidak mengalami Drop Out kontrasepsi (53,5%). Analisis statistik menunjukkan terdapat hubungan yang signifikan antara paritas dengan kejadian Drop Out kontrasepsi suntik 3 bulan (p = 0,002; OR = 0,132), yang mengindikasikan bahwa ibu multipara memiliki peluang lebih rendah mengalami Drop Out dibandingkan ibu primipara. Temuan ini memberikan bukti bahwa paritas merupakan salah satu faktor yang berperan dalam keberlangsungan penggunaan kontrasepsi DMPA dan dapat menjadi dasar penguatan skrining serta konseling KB yang lebih terarah sesuai karakteristik paritas akseptor.
