PROSEDUR PEMERIKSAAN RADIOGRAFI BIPOLAR URETROCYSTOGRAFI DENGAN KLINIS STRIKTUR URETRA DI RSI SULTAN AGUNG SEMARANG

ABSTRACT
Urethral stricture is a narrowing of the urethral lumen that can cause urinary problems, requiring bipolar urethrocystography to evaluate the location and length of the urethral narrowing. This study aimed to determine the examination procedure and the reasons for using 100 ml of contrast media in the urinary bladder and 30 ml in the urethra during bipolar urethrocystography in a case of urethral stricture at RSI Sultan Agung Semarang. This qualitative study was conducted at the Radiology Installation of RSI Sultan Agung Semarang from February to June 2026. The research subjects consisted of three radiographers and one radiologist, while the research object was the bipolar urethrocystography examination procedure for clinical urethral stricture. Data were collected through observation, interviews, and documentation, then analyzed narratively and compared with relevant theories to draw conclusions. The results showed that the examination was performed without special preparation because the patient had a cystostomy catheter in place. The examination used a combination of retrograde and antegrade techniques with a plain AP pelvis projection followed by post-contrast imaging using an RPO pelvis projection. The contrast media consisted of water-soluble iodine mixed with sterile water, with a volume of approximately 30 ml for urethrography and 100 ml for cystography. The radiographic results clearly demonstrated the urethra, urinary bladder, and location of the urethral narrowing in the posterior urethra, thereby supporting the evaluation of urethral stricture. In certain cases, additional projection variations may be considered to support a more comprehensive evaluation of the urethral anatomy and abnormalities.
ABSTRAK
Striktur uretra merupakan penyempitan pada lumen uretra yang menyebabkan gangguan berkemih, sehingga diperlukan pemeriksaan bipolar uretrocystografi untuk mengevaluasi lokasi dan panjang penyempitan uretra. Penelitian ini bertujuan untuk mengetahui prosedur pemeriksaan serta alasan digunakannya media kontras 100 ml pada vesica urinaria dan 30 ml pada uretra pada pemeriksaan radiografi bipolar uretrocystografi dengan klinis striktur uretra di RSI Sultan Agung Semarang. Penelitian ini menggunakan metode kualitatif dan dilakukan di Instalasi Radiologi RSI Sultan Agung Semarang pada bulan Februari 2026 hingga Juni 2026. Subjek penelitian terdiri atas tiga radiografer dan satu dokter spesialis radiologi, sedangkan objek penelitian adalah prosedur pemeriksaan bipolar uretrocystografi dengan klinis striktur uretra. Data dikumpulkan melalui observasi, wawancara, dan dokumentasi, kemudian dianalisis secara naratif dan dibandingkan dengan teori yang relevan untuk ditarik kesimpulan. Hasil penelitian menunjukkan bahwa pemeriksaan dilakukan tanpa persiapan khusus karena pasien telah terpasang kateter cystotomi. Pemeriksaan menggunakan kombinasi teknik retrograde dan antegrade dengan proyeksi plain foto AP pelvis dan post kontras menggunakan proyeksi RPO pelvis. Media kontras yang digunakan sebanyak 30 ml pada uretra dan 100 ml pada vesica urinaria. Hasil radiograf mampu memperlihatkan uretra, vesica urinaria, dan lokasi penyempitan uretra pada pars posterior sehingga dapat mendukung evaluasi striktur uretra. Pemeriksaan radiografi bipolar uretrocystografi pada kasus striktur uretra dilakukan dengan pemasukan media kontras iodine water-soluble yang dicampur water steril melalui teknik retrograde dan antegrade. Pada kasus tertentu, penambahan variasi proyeksi dapat dipertimbangkan untuk mendukung evaluasi anatomi dan kelainan uretra secara lebih lengkap.
