ABSTRACT
Health policy at the hospital level serves as a strategic instrument for ensuring service quality and patient safety. Despite the implementation of various regulations and accreditation standards, numerous reports indicate persistent discrepancies between formulated policies and actual practices in healthcare settings, creating an implementation gap that may affect the effectiveness of hospital governance. This article aims to comprehensively analyze the hospital policy process from formulation and implementation to evaluation. The study employs a narrative literature review approach in health policy through the identification, selection, and critical analysis of relevant scientific publications, regulations, and policy documents to obtain a comprehensive understanding of the dynamics of hospital policy implementation. The findings reveal that policy success is not determined solely by administrative directives but rather by the synergy between adaptive top-down and bottom-up approaches. Furthermore, the effectiveness of policy implementation is influenced by organizational capacity, healthcare workforce engagement, the quality of internal communication, and sustainable evaluation mechanisms. Strategic recommendations include strengthening ex-ante risk management, utilizing Cost-Effectiveness Analysis (CEA) as an evaluation instrument, and institutionalizing an open organizational culture (just culture) to ensure the sustainability of service quality. These findings underscore that evidence-based policies, adaptability to change, and strong organizational culture are essential prerequisites for the continuous improvement of healthcare quality and patient safety.
ABSTRAK
Kebijakan kesehatan di tingkat rumah sakit merupakan instrumen strategis untuk menjamin mutu pelayanan dan keselamatan pasien. Meskipun berbagai regulasi dan standar akreditasi telah diterapkan, berbagai laporan menunjukkan masih ditemukannya ketidaksesuaian antara kebijakan yang dirumuskan dengan praktik pelayanan di lapangan, sehingga memunculkan kesenjangan implementasi (implementation gap) yang berpotensi memengaruhi efektivitas tata kelola rumah sakit. Artikel ini bertujuan untuk menganalisis secara komprehensif alur kebijakan rumah sakit dari tahap perumusan, implementasi, hingga evaluasi. Penelitian ini menggunakan metode tinjauan naratif literatur kebijakan kesehatan melalui identifikasi, seleksi, dan analisis kritis terhadap berbagai publikasi ilmiah, regulasi, serta dokumen kebijakan yang relevan guna memperoleh pemahaman yang komprehensif mengenai dinamika implementasi kebijakan rumah sakit. Hasil analisis menunjukkan bahwa keberhasilan kebijakan tidak ditentukan oleh instruksi administratif semata, melainkan melalui sinergi antara pendekatan top-down dan bottom-up yang adaptif. Selain itu, efektivitas implementasi kebijakan dipengaruhi oleh kapasitas organisasi, keterlibatan tenaga kesehatan, kualitas komunikasi internal, dan mekanisme evaluasi yang berkelanjutan. Rekomendasi strategis mencakup penguatan manajemen risiko ex-ante, penggunaan instrumen evaluasi Cost-Effectiveness Analysis (CEA), serta pelembagaan budaya organisasi yang terbuka (just culture) untuk menjamin keberlanjutan mutu pelayanan. Temuan ini menegaskan bahwa kebijakan rumah sakit yang berbasis bukti, adaptif terhadap perubahan, dan didukung budaya organisasi yang kuat merupakan prasyarat penting bagi peningkatan kualitas layanan dan keselamatan pasien secara berkelanjutan.
