DINAMIKA KONFLIK PSIKODINAMIK, POLA ASUH, DAN STRESOR PSIKOSOSIAL PADA KASUS AGORAFOBIA

This qualitative study utilizes a case study approach that combines observational and biographical studies. The subject is a 26-year-old Balinese woman with a high school education, unmarried, and self-employed. The patient was diagnosed with agoraphobia and panic disorder, reporting anxiety that arises in crowded places, public settings, while waiting in line, or when alone at home. She had experienced these symptoms for three years, with significant worsening in the five months before she visited the psychiatric clinic. This condition has subsequently limited her daily activities and work. The findings of this study indicate that the patient’s mental health disorder stems from complex internal conflicts involving the interplay of biopsychosocial and cultural factors. Ambivalent parenting styles, economic difficulties and prolonged parental relationship conflicts contribute to the patient’s mental health condition. If left unmanaged, these issues may lead to additional problems and stressors for both the patient and her family. Overall, this case underscores the importance of understanding psychodynamic conflicts rooted in parenting patterns, family environment, and psychosocial stressors in the diagnosis and treatment of agoraphobia. It enriches the clinical perspective in designing holistic therapeutic interventions.
ABSTRAK
Penelitian ini merupakan penelitian kualitatif melalui pendekatan studi kasus menggabungkan studi observasi dan studi biografi. Seorang perempuan 26 tahun, Bali, pendidikan SMA, belum menikah, wiraswasta. Pasien didiagnosa dengan agorafobia dengan gangguan panik, mengeluhkan cemas yang muncul ketika berada di keramaian, di tempat umum, sedang mengantri atau ketika di rumah sendirian. Hal ini sudah dialami selama 3 tahun ini dan memberat dalam 5 bulan sebelum datang ke poliklinik jiwa. Kondisi ini kemudian membatasi aktivitas dan pekerjaan pasien. Hasil dari penelitian ini adalah bahwa gangguan jiwa yang dialami pasien merupakan akibat dari konflik internal yang kompleks, yang melibatkan interaksi faktor biopsikososiokultural. Pola asuh ambivalensi, masalah ekonomi, konflik hubungan orang tua yang berkepanjangan berperan dalam kondisi gangguan jiwa yang dialami pasien. Hal ini jika tidak dikelola dengan baik maka akan memunculkan masalah dan stresor baru bagi pasien dan keluarga. Secara keseluruhan, kasus ini menyoroti pentingnya pemahaman terhadap konflik psikodinamik, yang berakar pada pola asuh, lingkungan keluarga, dan stressor psikososial dalam diagnosis serta penanganan agorafobia. Hal ini memperkaya perspektif klinis dalam merancang intervensi terapeutik yang holistik.
