TATALAKSANA PEMBEDAHAN UNTUK INFANTIL PERIANAL PYRAMIDAL PROTRUSION (IPPP): SEBUAH LAPORAN KASUS LANGKA

Infantile Perianal Pyramidal Protrusion (IPPP) is a rare and benign condition commonly observed in children under five years of age. Although typically self-resolving with conservative management, surgical intervention may become necessary in persistent cases. This report presents a 2-year-old girl with IPPP that failed to respond to conservative treatment and required surgical excision. A 2-year-old girl presented with a solitary perianal protrusion observed for over a month. Conservative treatments, including dietary changes, laxatives, and anti-inflammatory medications, were unsuccessful. The lesion increased in size over six months, necessitating surgical removal. Post-operative care included wound management, pain control, and antibiotics. The wound healed uneventfully, and the child remained asymptomatic. First described by Kayashima in 1996, IPPP is characterized by a pyramidal soft tissue protrusion in the midline perianal region. Conservative management typically resolves functional types associated with constipation or irritation. Surgery is reserved for cases unresponsive to non-invasive approaches. IPPP is benign and has an excellent prognosis. Conservative treatment is the first line, but surgery may be indicated for persistent cases.

Infantile Perianal Pyramidal Protrusion (IPPP) adalah kondisi langka dan jinak yang umum terjadi pada anak-anak di bawah usia lima tahun. Meskipun biasanya dapat sembuh sendiri dengan penanganan konservatif, intervensi bedah mungkin diperlukan dalam kasus yang terus-menerus. Laporan ini menyajikan seorang anak perempuan berusia 2 tahun dengan IPPP yang gagal merespons pengobatan konservatif dan memerlukan eksisi bedah. Seorang anak perempuan berusia 2 tahun datang dengan tonjolan perianal soliter yang diamati selama lebih dari sebulan. Perawatan konservatif, termasuk perubahan pola makan, pencahar, dan obat antiradang, tidak berhasil. Lesi bertambah besar selama enam bulan, sehingga memerlukan operasi pengangkatan. Perawatan pascaoperasi meliputi penanganan luka, pengendalian nyeri, dan antibiotik. Luka sembuh tanpa masalah, dan anak tersebut tetap asimtomatik. Pertama kali dijelaskan oleh Kayashima pada tahun 1996, IPPP ditandai dengan tonjolan jaringan lunak berbentuk piramida di daerah perianal garis tengah. Penanganan konservatif biasanya mengatasi jenis fungsional yang terkait dengan konstipasi atau iritasi. Pembedahan hanya dilakukan untuk kasus yang tidak responsif terhadap pendekatan non-invasif.
